PM45 Prism Cold Shoe Replacement

I have a PM45 Prism and the cold shoe has a crack in it.  I use this for the magnifier accessory, so I need to replace the cold shoe.  Two questions:
1. Where can I find a replacement cold shoe (mine has 2 screws and then a black u-shaped insert)?
2. When the two screws are removed, the shoe still does not come loose.  How do I remove the cold shoe?
