Nick,
I've been using extensis portfolio for 9yrs.
It's basically an image library with a great search engine.
I keep the 260.000 images written on the dvds cataloged
and simple to find.
Don't know if that's what you're looking for, but you can

DVD's ? thats a lot of potential drink coasters!  how do you ensure they are all readable?  with the plummeting cost of HDD's I would have thought it's time to migrate? and how much space do DVD's take up - but that's another thread

Extensis has been round a long time the new kids on the block Lightroom and Aperture are DAM systems with that little bit extra

We are using Aperture2 not up to that no of images for at least another year or so, as annotation and key wording take time  but images are easy to find  on multiple search fields, even those whose Master image files are not in the live archive as disk not connected only 1TB of Master Files live at moment (external eSATA array is next major move) 
Aperture has the advantage of a large developer/user base and is relatively cheap it just needs a Phocus plug in.

Some academic institutions are using  http://www.lunaimaging.com/  which is rather expensive
