28mm HCD Aperture problem

Already posted this on Flexframe, so apologies if you're reading this twice.

Was shooting on location today when I noticed that I was getting inconsistent exposures. Turned out that the aperture on my 28mm HCD was being inconsistent and on occasions it was only partially stopping down (either during a shot or using the Stop Down button). Anyone else seen this? What turned out to be the problem and the fix?
